Music Centre Registration Forms

We require all Tuesday, Wednesday & Thursday music centre members (including 19 yrs/Y14+) to fill out both our Music Centre registration form, and our photo and video permissions form.  

Copies of both forms are available to download here:

- Photo and Video Permissions Form 

When complete, the forms should be signed then returned to our Music Administrator Lizzie Sale, or a Music Centre Staff Member.
